Wickiup Residence Hall Student Vaccination Attestation Form

Residence Hall students are required to be fully vaccinated against COVID-19 prior to moving into Wickiup Hall. CDC guidelines recommend boosters for those whose full vaccination was completed at least five months ago. COCC will require boosters if the last vaccination dose occurred more than five months ago. Individuals can request a vaccination exemption per Oregon State law, including if you received the vaccination and do not intend to receive the booster. All unvaccinated or not-yet-fully-vaccinated residents who request the exemption will be required to partake in weekly COVID-19 testing, per COCC's COVID-19 testing policy. These COVID-19 tests are free and will be provided by the College.
