- Confirmation of Health Screening Form (physician's endorsement to participate in field research)
- TB Test Results form
- Acknowledgement Regarding Travel Insurance Form (as required by COCC for participation)
- Acknowledgement of Risk and Consent for Treatment Form

- Standards of Conduct for Study Abroad Form
- Release Form-Central Oregon Community College
- Release Form-Pisat Stido Satwa Primata-Institut Pertanian Bogor
- Photo/Video Consent Form-COCC
- $25 Application fee (non-refundable)
