Eligibility and Requirements

Participants be at least 18 years of age and have a valid passport.

Participants must be prepared to submit proof of Covid-19 immunization and/or Covid-19 tests at time of travel as required by host government. Current Indonesian entry requirements can be found at the US Embassy Website.

Participants must submit a completed application packet to include:

Personal Information Data Sheet
Three Letters of Reference
Confirmation of Health Screening Form (physician's endorsement to participate in field research)
TB Test Results form
Acknowledgement Regarding Travel Insurance Form (as required by COCC for participation)
Acknowledgement of Risk and Consent for Treatment Form
