Nursing Advanced Placement Application

Steps for applying to the Second Year of the COCC Nursing Program as an Advanced Placement (LPN to RN) Student:

1. Submit Transcripts - deadline February 1^st*

Send COCC an official copy of all of your Nursing coursework transcripts (including pre-requisite courses) along with the Transcript Evaluation Request/Nursing File Submission Form.
Write "Advanced Placement Applicant" on the top of the form and mail to:

Within 2-3 weeks, you will recieve a response sent to your email letting you know what classes you will be given credit for, and what you still need to take before entering the program.

2. After you recieve your response, call 51-383-7200 to schedule an appointment with the Pre-Nursing Advisor - deadline March 1^st*

Please bring the following completed documentation to your advising appointment:

Nursing Program Advanced Placement Application - page 15 of the Nursing Program Readmission, Advanced Placement and Transfer Student Policies packet
Verification of Employment - page 14 of packet - 400 hours of direct patient care in an acute or long-term (skilled) setting, or 600 hours of direct patient care in other nursing sttings such as clinics, home health, or assisted living as an LPH withing the last two years.
Current State of Oregon LPN license number. Your license must be active and unencumbered. License will be verified through the Oregon State Board of Nursing.

3. Meet with the Nursing Program Team II Coordinator - deadline March 15^th*

You will be given the contact information at your advising appointment. The Coordinator will schedule you for:
- HESI-PN exam - must pass at ≥ 850 points - deadline May 31^st*
- Comprehensive skills assessment test - deadline May 31^st*

Note: *Students must meet these deadlines to be guaranteed consideration for admittance to the Fall Second Year Nursing Cohort. Transcripts will be accepted and advising may be completed after those dates, but the students will be placed at the back of the consideration list. Testing will not be conducted after May 31^st.
