Traditional Nursing Program Application 2014
Prepare to Apply - Students who are nearing completion of the Nursing Pre-requisite coursework should prepare by:
- Review 2013 Selection Process Handbook - link
- Attend a Nursing Orientation if you have any questions - link
- Check Degree Audit in GradTracks
- Verify pending pre-requisite completion and documentation of all transfer coursework in GradTracks.
- Submit Transcripts if transfer coursework is not documented in GradTracks.
- Transcript Evaluation Request/Nursing File Submission Form - have transcripts sent to your home; leave in sealed envelope, and submit with form to Enrollment Services, Boyle Education Center
- Confirm Certified Nursing Assistant Requirement completion documented
- Nursing 95 completed at COCC or equivalent transferred in from another Oregon Community College
- Verify completion documented in GradTracks with grade of "C" or higher
- Submit Transcripts if transfer coursework is not documented in GradTracks
- Transcript Evaluation Request/Nursing File Submission Form - have transcripts sent to your home; leave in sealed envelope, and submit with form to Enrollment Services, Boyle Education Center
- Or submit Oregon CNA license number via email to sbarry@cocc.edu
- Certified Nursing Assistant Class taken elsewhere
- If course will be completed on or before March 22, 2013
- Submit Oregon CNA license number via email to sbarry@cocc.edu if needed - see Selection Process Handbook for details
- If course is completed between March 22, 2013 and June 17, 2013
- Submit a copy of your Certificate of Completion with Transcript Evaluation Request/Nursing File Submission Form before 5pm June 17, 2013
- Then Email Oregon CNA Level 1 license number to sbarry@cocc.edu before August 15, 2013
- TEAS-V Exam
- Study for, schedule and take the TEAS-V Exam at COCC

- Deadline for submission will be Monday, June 17th, 2013 before 5pm.
- Short Answer Questions
- Answers are submitted with the application.
- The 2013 Short Answer Questionswill be available here mid-March 2013.
