2013 Traditional Nursing Program Application Information

Prepare to Apply

Students who are nearing completion of the Nursing pre-requisite coursework should:

1. Review the 2013 Selection Process Handbook

2. Attend a Nursing Orientation

3. Submit official transcripts for any prerequisite coursework taken at other colleges or universities to the Nursing Application File

• This is in addition to any transcripts you may have submitted to the COCC Enrollment Services File
• Official transcripts must be received by 5 pm June 17, 2013
  • Students who are enrolled in prerequisite coursework at other colleges the Spring term they are applying should consult with the Pre-Nursing Advisor regarding timely submission of those transcripts 
   
• You do not need to submit your COCC transcript
• Transcript submission process: 
  •  Have official transcript(s) sent to your home - leave in sealed envelope(s)
  •  Attach the sealed envelope(s) to the Transcript Evaluation Request/Nursing File Submission Form
   

Submit form and transcript(s) together to: 

 4. Submit documentation showing completion of the Certified Nursing Assistant application prerequisite to the Nursing Application File - must be submitted by 5 pm June 17, 2013

• If you completed or will complete Nursing 95 at COCC between Fall 2008 and Spring 2013 with a grade of "C" or higher
  • No action is necessary
   
• If you completed a course equivalent to Nursing 95 at another Oregon Community College between Fall 2008 and Winter 2013 with a grade of "C" or higher
  • Submit transcript to Nursing Application File by 5pm June 17, 2013 – see step 3 above
  • Or, if you have a current Oregon CNA license, email your license number to sbarry@cocc.edu by 5 pm June 17, 2013
   
• If you are currently taking or have completed a Certified Nursing Assistant Class at an unaccredited agency (i.e., nursing home or hospital)
  • If course will be completed on or before March 22, 2013
    • You must take and pass the Oregon CNA-1 licensing exam and be issued a valid Oregon CNA-1 license number
    • Email your Oregon CNA license number to sbarry@cocc.edu by 5pm June 17th
    •  See Selection Process Handbook for details  
     
  • If course will be completed between March 22, 2013 and June 17, 2013
    • Submit a copy of your Certificate of Completion with the Transcript Evaluation Request/Nursing File Submission Form by 5 pm June 17, 2013
    • You must then take and pass the Oregon CNA-1 licensing exam and be issued a valid Oregon CNA-1 license number by August 15th, 2013
    • The email your Oregon CNA license number to sbarry@cocc.edu by 5pm August 15, 2013
     
   

5. Prepare for the TEAS-V Exam

• Study for the TEAS-V
• Schedule TEAS-V testing date(s)
• Information is available on the TEAS-V: Nursing Program Admission Testing web page

6. Prepare Short Answer Questions

• Review the 2013 Short Answer Information and Guidelines

• Review questions and prepare answers
  •  2013 Short Answer Questions 
  •  Prepare your answers in a Word Document prior to starting the Application.  This will allow you to easily cut and paste your answers into the Application. 
   

Apply

1. Take TEAS-V Exam at COCC before June 17^th, 2013

2. Complete Application and submit answers to the Short Answer Questions

• 2013 Application is available online  
• Deadline for submission of the Application and answers to Short Answer Questions is 5 pm Monday, June 17, 2013
• Answers to the Short Answer Questions are submitted online within the Application
• The Application may be submitted before or after taking the TEAS-V.  They must both be completed by Monday, June 17, 2013
